Mobile top-up translation help!

Hi - I am trying to top up a Turkcell mobile and before I commit by credit card details (!!) wanted to check I had understood site okay.

 

This is what it says - and what I think it means....

 

  - phone number to be topped up
  - amount of top-up
name on credit card (firstname then last name)
credit card number
CVV*   expiry date and pin
  type of card

*Kartınızın arkasında bulunan 3 haleni güvenlik numarasıdır. - explaining where to find CVV number

 

Could someone confirm that I have understood it okay please.

You are right in all aspects, except that CVV is not your 4 digit PIN no, it is the 3 digit code written on the signature strip on the reverse of the card.
