Can somebody help me translating the following into Turkish?

1 - ´The accordance with the registrar is certified´ -

- kayıt ile uygun sertifikalı?

2 - ´It is hereby certified that´ (Mr. X)

- isbu sertifikalı? 

3 - ´..according to Polish Law, fulfils the requirements for entering into marriage   with´ (Mrs. X)

     I´m lost!            

4- ´The latter has not been subject to verification of the conditions for marriage under Polish Law´

     again lost!

5 - ´Issuing authority´ 

- emisyon makamı?

 Emisyon means emmissions like the exhaust of a car.

İlgili Makamı would be better

´It is hereby certified that´ (Mr. X)
