"Sen" is informal and singular. If you are talking to a close friend, you use "sen". For example: "Ahmet, sen burada mıydın?" That's simple.

"Siz" is formal and/or plural.
1) When talking to more than one person - be it two or more of your friends or teachers - you use "siz". For example: "Ahmet, Aslı, siz burada mıydınız?"

2) When talking to your teacher at school, you use "siz". Example: "Hocam (öğretmenim), siz burada mıydınız?"
