Denizlere çıkar sokaklar- yes it mean streets leads to seas.

But in this song it is reffering to Deniz Gezmiş and his friends which is called shortly Denizler, turkis revolutionist who executed.

And song is about new 68 generation, people ll be folllow Denizler´s lead and they will create storm again, walk side by side at the streets etc...

So last sendence also mean then ´This paths leads us to Denizler´(to their ideas,actions etc). But when you translate it you can not keep wordplay..
