Aslında "rica ederim" kalıbının "lütfen" gibi bir anlamı daha var: Rica ederim artık gürültü yapmayın!
In fact, "rica ederim" has one more meaning like "please". Example: Rica ederim artık gürültü yapmayın! Please don't make noise anymore!

It is incorrect. Yes it can mean please but stil that sentence is incorrect.
"Sütünüz var mı rica etsem?" could be alright. Sound a little forced but is alright.
"Rica etsem" is a polite way of requesting.
"Rica etsem pencereyi kapatabilir misiniz?" is strong.

i guess it depends who you are talking to, if it's someone you know well it would be ok to you 'misin' i think.

"misiniz" is polite but "misin" too,
"rica etsem gelir misin?"= "could you come?"
polite form is in adding "rica etsem" words.
Bir bardak su verir misin rica etsem?
Bir bardak su verir misin lütfen?

Rica means a request, so rica ederim means "I requested".
I have heard this used a lot, my husband uses it in restaurants, it confused me at first. However if you think of the "your welcome" meaning more as "it was a pleasure" ie "I did what I wanted to do" then the "want" part can easily be understood.
"bir ayran rica ederim" = "I 'want- request' an ayran",
As a, "it's a pleasure" "rica ederim" = "I do that which I wanted to do".
I hope this does not confuse you further, I am trying to help, but I re-read it and I am not sure wether I have expressed myself clearly.
