Could someone explain the R in şismanlarsiniz please

 Şişmanlamak = to get fat

 Şişmanla [ Root of the verb] + r [suffix for simple present tense] +sınız [ ending for person,you plural]

So " R" here is suffix for simple present tense..

You will be fat

You may get fat (if you do ...)

you get fat (as a result of ...)

etc.

Edit:

Also you can parse it like this:

Şişmanlar = fat people

şişmanlarsınız = you are (a bunch of) fat people
