it should be "unutma" though. The rest seems okay to me, but I am also learning, so better wait for other replies.

"unutma" means "don't forget", but it is written "remember" in the text, and "kendine dikkat et" means "take care of yourself", but it is written "be careful" which means "dikkatli ol". so the rest of the sentence should be like this:
Lütfen dikkatli ol ve seni daima sevdiğimi hatırla.

Some translate "I liked it very much" as "ondan çok hoşlandım". You can say about somebody who you liked "ondan çok hoşlandım" but not about a talk. A talk is a talk. No one would call a talk "o". I just wanted to make a comment. So my version of translation is as follows,
Bu akşamki sohbet için teşekkürler. çok keyif aldım. Lütfen dikkatli ol ve hatırla ki seni hep seveceğim.
